MEDICAL RECORDS There are times when you may need a copy of your medical records. Because of confidentiality issues and HIPPA law, there is a form to release the information for you to sign so this can not be accomplished over the telephone. There may be fees involved so be sure to inquire when requesting forms from our office.
How often can I get a copy of my medical records? Federal and State laws allow everyone to have one copy of their own medical records. If you want additional copies there will be a charge of $1.00 per page payable before copies are released. Is there a charge to fill out disability papers? Disability forms vary in structure and there could be a charge up to $20.00. If you need these filled out please ask ahead of time if there will be a charge. What about Workers Compensation forms?
